ABSTRACT

Burkitt's lymphoma, a form of non-hodgkin lymphoma, is a neoplastic monoclonal proliferation of lymphoid cells in areas of the immune system. it can occur in HIV-positive patients, as AIDS is related to the development of non- hodgkin lymphoma. burkitt's lymphoma is a rare subtype, highly prevalent in patients with AIDS. incisional biopsy, in situ hybridization and computerized axial tomography are the appropriate tests to determine the characterize of the lesions. the case of a 4-year-old HIV-positive patient, who developed burkitt's lymphoma of the oral cavity, is reported in this paper. the aim of this case report is to describe the course of the pathology, taking into account its clinical imaging characteristics and treatment.

ABSTRACT

ABSTRACT Jawbone reconstruction after tumor resection is one of the most challenging clinical tasks for maxillofacial surgeons. Osteogenic, osteoinductive, osteoconductive and non-antigenic properties of autogenous bone place this bone as the gold standard for solving problems of bone availability. However, the need for a second surgical site to harvest the bone graft increases significantly both the cost and the morbidity associated with the reconstructive procedures. Bone grafting gained an important tool with the discovery of bone morphogenetic proteins in 1960. Benefit of obtaining functional and real bone matrix without need of second surgical site seems to be the great advantage of use bone morphogenetic proteins. This study analyzed the use of rhBMP-2 in unicystic ameloblastoma of the mandible, detailing its structure, mechanisms of cell signaling and biological efficacy, in addition to present possible advantages and disadvantages of clinical use of rhBMP-2 as bone regeneration strategy.


RESUMO A reconstrução óssea dos maxilares após ressecções tumorais é uma das tarefas mais difíceis para o cirurgião maxilofacial. As propriedades osteogênicas, osteoindutoras, osteocondutoras e não antigênicas do osso autógeno o colocam como o padrão-ouro para a solução de problemas de disponibilidade óssea. Entretanto a coleta do enxerto ósseo necessita de um segundo sítio cirúrgico, aumentando significativamente o custo e a morbidade associados ao procedimento reconstrutivo. A enxertia óssea ganhou uma excelente ferramenta com a descoberta das proteínas ósseas morfogenéticas na década de 1960. O benefício da obtenção de matriz óssea verdadeira e funcional, sem a necessidade de um segundo sítio cirúrgico, parece ser a grande vantagem do uso das proteínas ósseas morfogenéticas. Neste contexto, o objetivo deste estudo foi analisar a utilização da rhBMP-2 na regeneração óssea de ameloblastoma mandibular unicístico, detalhando sua estrutura, seus mecanismos de sinalização celular e sua eficácia biológica, além de apresentar potenciais vantagens e desvantagens da utilização clínica das rhBMP-2, enquanto estratégia regenerativa.

ABSTRACT

Prostate cancer represents the most frequent non-cutaneous neoplasia in males. This type of neoplasia can develop peculiar patterns of evolution, presenting, in many cases, precocious relapses and metastasis. Bone metastasis in the mouth is extremely rare, and represents 1% of all malignant mouth neoplasias. The aim of the present study is to report a clinical case of bone metastasis in the mandibular region associated with a tumoral prostate adenocarcinoma, as well as to discuss connected aspects about diagnosis, prognosis and integrated treatment of this condition.

ABSTRACT

The use of fine-needle aspiration biopsy (FNAB) in the diagnosis of odontogenic tumors seems to have attracted little attention. The presence of a firm preoperative diagnosis helps preventing suboptimal surgery, contributing to avoid recurrence of these tumors. A case of ameloblastic carcinoma of the mandible diagnosed by FNAB is presented in this report, illustrating its effectiveness for preoperative diagnosis of odontogenic tumors. A 74-year-old female presented with a painless swelling in the right mandibular angle. A panoramic radiograph revealed a radiolucent lesion in the body of the mandible. Cytological smears from FNAB in the area revealed basaloid cells with a palisade arrangement and presence of stellate-shaped cells. These cytological features lead to the diagnosis of ameloblastoma. However, when there are atypical cells and atypical mitoses, as in the present case, diagnosis of ameloblastic carcinoma may be established. The patient underwent chemotherapy, showing remission of the lesion after treatment. FNAB is a minimally invasive, safe, fast and inexpensive method for diagnosing benign and malignant ameloblastomas, which ensures that patients have a proper treatment without the need of performing an incisional biopsy, especially in neoplastic cases.


A punção aspirativa por agulha fina (PAAF) para o diagnóstico dos tumores odontogênicos tem atraído pouca atenção. A presença de um diagnóstico pré-operatório ajuda a prevenir tratamentos sub-ótimos e evita recidivas no caso destes tumores. Um caso de PAAF de carcinoma ameloblástico é apresentado, ressaltando a eficácia deste método para o diagnóstico pré-operatório dos tumores odontogênicos. Paciente feminina, de 74 anos, com abaulamento indolor na região mandibular à direita. O RX panorâmico mostrou lesão radiolúscida no corpo da mandíbula. Realizada PAAF da área, cujos esfregaços citológicos mostraram arranjo em paliçada de células basalóides e presença de células de aspecto estrelar. Tais características citológicas levam ao diagnóstico de ameloblastoma, porém na presença de atipias celulares e de mitoses atípicas, como no presente caso, o diagnóstico deve ser de carcinoma ameloblástico. Após o diagnóstico a paciente foi submetida à quimioterapia. A PAAF é um método minimamente invasivo, seguro, rápido e barato para o diagnóstico de ameloblastomas benignos e malignos, que permite que os pacientes tenham um tratamento apropriado sem a necessidade de biópsia incisional, especialmente em casos neoplásicos.

ABSTRACT

PURPOSE: To investigate the effects of bisphosphosnate alendronate (ALD) and metotrexate (MTX) on an experimental model of Walker 256 carcinosarcoma developed in the oral cavity of rats. METHODS Walker 256 carcinosarcoma cell suspension (0,1 mL) containing 10(6) cell/mL was implanted in the alveoli of the first and second molars. The animals were divided and treated with saline, MTX, ALD, and MTX plus ALD. Later, the animals were sacrificed, the tumors were measured and the mandibles were removed for radiographic and histological analysis. RESULTS: In the control group, the radiographic images demonstrated radioluscency with poorly defined borders, and the microscopic examination revealed tumor infiltration into the peripheral and central regions of the bone. Areas of necrosis were commonly seen. In the treated groups with ALD, associated or not with MTX, the radiographic analysis revealed circumscribed tumor-induced osteolysis and various degrees of radiotransparence; while, histologically, preserved bone trabeculae with osteoid formation was observed among malignant cells. CONCLUSION: The bisphosphonate alendronate exherted an osteoprotective effect and induced bone neoformation on the Walker 256 carcinosarcoma inoculated in rat mandibles. The combination of metotrexate with bisphosphonate alendronate is more successful than treatment with the agents alone in controlling the growth of neoplastic cells and in stimulating reactive new bone. Therefore, this may be an alternative treatment to malignant lesions of maxillaries with osteolysis.


OBJETIVO: Avaliar o efeito do bisfosfonato alendronato (ALD) e do metotrexato (MTX) em modelo do carcinossarcoma 256 de Walker na mandíbula de ratos. MÉTODOS: Uma suspensão de células tumorais do carcinossarcoma 256 de Walker (0,1mL), na concentração de 10(6) células/mL, foi implantada nos alvéolos de ratos previamente abertos por exodontia. Os animais foram divididos em grupos e tratados com salina, MTX, ALD e associação do MTX com ALD. Após o sacrifício, os tumores foram medidos e as mandíbulas removidas para exames radiográfico e histológico. RESULTADOS: No exame radiográfico do grupo controle foi verificada área lítica, sem evidência de reparo na região dos alvéolos, e no microscópico, infiltração óssea periférica e central, de pequenas células tumorais com diversas áreas de necrose. Nos grupos tratados com ALD, associado ou não ao MTX, a análise radiográfica mostrou redução da osteólise tumor-induzida com graus variados de radiotransparência na estrutura óssea; enquanto que, na análise histopatológica, trechos de tecido ósseo preservado, com formação de osteóide em meio a células tumorais foram observados. CONCLUSÃO: O bisfosfonato alendronato exerceu um efeito osteoprotetor e induziu neoformação óssea em mandíbulas de ratos implantadas com células de carcinossarcoma de Walker 256. A combinação do metotrexato com o bisfosfonato alendronato foi mais eficaz que o tratamento com os agentes isoladamente no controle do crescimento das células neoplásicas e na estimulação da formação óssea. Sendo assim, essa associação constitui-se uma alternativa de tratamento das neoplasias malignas dos maxilares com invasão e comprometimento ósseo.
